Regence BlueShield

Regence BlueShield is offering several different comprehensive and catastrophic plan designs. These plans have 3 level of benefits depending on which network you use. 

Category 1: Preferred Providers. You’ll generally have lower out-of-pocket costs when you see Preferred Providers.
 
Category 2: Participating Providers. When you see Participating Providers you’ll generally pay more out-of-pocket. 
 
Category 3: Non-contracted Providers. You’ll have the highest out-of-pocket costs when you see these providers. These providers have not agreed to negotiated rates with Regence so members may be responsible for costs beyond the pre-determined amount.  

All plans provide 24 hour on the job coverage.

Service Plans

Evolve Core

Evolve Core is a catastrophic health plan with your choice of 4 deductible options and 70% coinsurance when you use preferred providers.  The plan provides four office visits with the deductible and coinsurance waived; you only pay the $35 office copay.  Preventive care is covered at the coinsurance level with the deductible waived. The first $200 of lab and x-ray are covered in full with the deductible and coinsurance waived. These plans do not include maternity, vision, or prescription drug benefits.  

Evolve Plus

Evolve Plus is a comprehensive health plan with your choice of 4 deductible options and 80% coinsurance when you use preferred providers.  The plan provides four office visits with the deductible and coinsurance waived; you only pay the $25 office copay.  Preventive care is covered at the coinsurance level with the deductible waived. The first $400 of lab and x-ray are covered in full with the deductible and coinsurance waived. This plan also includes maternity, vision exam/hardware, and prescription drug.

Evolve HSA

The HSA plan offers a $2,000 or $3,500 individual deductible and a $4,000 or $7,000 family deductible medical plan with your choice of 80% or 50% coinsurance.  The deductible is waived for preventive care. These plans do not include maternity or vision. 

Evolve HSA 100 Plan

The HSA 100 plan has a $5,000 individual deductible and a $10,000 family deductible. The plan offers 100% coinsurance after the deductible is met. This plan does not include maternity or vision.

Evolve Dental

Evolve dental options are available only to individuals and families who purchase Regence Evolve medical plans during the initial enrollment.  
 

Option 1

This plan offers $750 annual maximum benefit. Preventive care is covered at 100% with the deductible waived. Basic services are covered at 80% and Major services at 50% subject to the deductible. The $750 annual maximum may increase on a rewards basis (For example if you have at least one claim and incur less than $500 in claims Regence will add $250 to next year’s annual maximum.) There is a 6 month waiting period for Basic services and 12 months for Major. 
 

Option 2

This plan has no deductibles, a $750 annual maximum and preventive, basic and major services are combined. The first $200 of dental expenses are covered at 100%. After the first $200 all expenses are covered at 50%.
