Plan Options

Balance Plan

Balance Plans have two comprehensive plans that include maternity and prescription drugs and two catastrophic plans. Regardless of whether you use the in-network (Alliant Plus) or out-of-network providers (any other licensed provider) your deductible, coinsurance and office copays will remain the same. If you use the in-network providers your deductible is waived for office visits so you only have the office copay. The deductible is also waived out-of-network for preventive care, vision exams and vision hardware.

Welcome Plans

Welcome Plans are the HMO plan designs, which require you use Group Health providers. These plans offer 5 visits up front with the deductible waived. Upon your 6th visit you are subject to the deductible and coinsurance.

HealthPays HSA

HealthPays HSA plan has a $2,000 individual deductible or a $4,000 family deductible with prescription drug coverage and the deductible waived for preventive care. If you use Alliant Plus providers the plan covers you at 90%, out-of-network is 80%. The plan does not include maternity or vision coverage. You can set up a HSA with any financial institution of your choice; two that have been popular are HSA Bank and Wells Fargo.
