With Assurant Health you have a wide variety of plans offering comprehensive, catastrophic or HSA plans. These plans provide direct access to providers within the First Choice Network, so referrals are not necessary. All plans provide worldwide coverage, initial rate guarantees, patient care advocate, and 24 hour on the job coverage.

Service Plans
Assurant Comprehensive
Assurant Comprehensive plan offers you a $1,500 deductible and 75% coinsurance. You pay a $35 copay for office visits on your first 4 visits, then your are subject to the deductible and coinsurance. The plan also covers maternity, preventive, and prescription drugs ($2,000 max.). |
Assurant Catastrophic
Assurant Catastrophic offers coverage for major medical events as well as four office visits with a $35 copay and the deductible and coinsurance waived. Preventive care and mammograms are not subject to the deductible or coinsurance. Plans include an Accidental Medical Expense benefit, which pays 100% the first $2,000 of any accident related expense regardless of deductible. You customize your plan by selecting from a range of deductibles, coinsurance, out-of-pocket maximums, and lifetime maximums. Rx is not included; however Assurant offers an Rx card that you may purchase separately. |
Assurant Health Savings Accounts
This high deductible health plan has a $2,700 individual/$5,400 family in network deductible, with no prescription drug coverage. Preventive care is covered in full up to $500 without deductible or coinsurance. Assurant has teamed up with UMB Bank for an HSA account with no setup fee or monthly service fees, however you may elect to use another financial institution. |
