Freedom to make choices and rewards for taking care of yourself. That's the inspiration behind Regence Life & Health's two, new dental plans. These plans allow you to use any licensed provider however you will receive greater savings if you use a network provider.
Individual Dollar-Based Dental
This plan puts you in control of your dental health dollars. The plan is dollar-based, this means you can use your coverage almost any way you choose with few exclusions and limitations. Each year you decide to include an exam and cleaning, you are rewarded with a benefit increase the following year.
Plan features:
No Deductible
6 month waiting period for all covered services
$750 Annual Maximum (may increase annually if you qualify for the reward)
First $150 of care is covered at 100%
Next $500 of care is covered at 80%
Remaining care covered at 50%
You may choose any dentist but save more by using a network provider
Vision rider is available
Individual Incentive Dental
This is a more traditional plan, but unlike traditional dental plans you are rewarded for receiving routine preventive care. Each year that you visit the dentist for a checkup and cleaning, means greater benefits and less out-of-pocket expenses the next year.
Plan features:
No waiting period
Deductible waived for exams and cleanings
$50 deductible for other covered services
1st plan year benefits are covered at 80%/60%/30% (this may increase if you qualify for the rewards)
Choose any dentist, but save even more by using one of our network providers
Apply Online or to enroll by mail, complete the Enrollment Form and return it to us at the address below. If you are enrolling a domestic partner please return the Domestic Partner Affidavit with the application. Payment must be included with your application.
